If you are injured and need to submit a claim we will email the paperwork required from the information received in the form below.
ALL CLAIMS must be submitted to Industrial Alliance from WFTDi Canada on your behalf within 30 days. It is your responsibility to complete the form and submit it within this time frame.
Once the claim has been submitted and file number assigned all receipts are to be submitted to Industrial Alliance.
